- • Proof of Polycom Technology installation and maintenance or accreditation from EOM
- • Minimum two CVs of technicians with qualifications in Audio-Visual System Technology and Polycom Certification + Training
- • Fully completed SBD 1 (Invitation to Bid)
- • Quotation on company letterhead
- • Fully completed SBD 4 (Bidder's Disclosure)
- • Fully completed SBD 6.1 with original or certified B-BBEE Status Level Verification Certificate or B-BBEE Sworn Affidavit
- • Fully completed SBD 7.2 Contract Form
- • Proof of Central Supplier Database registration
- • Tax Compliance Status PIN or TCS Certificate
- • Awareness and compliance with OHS Act 85 of 1993
