- • Certified copy of Shareholders' certificates
- • Certified copy of ID documents of Directors/Members
- • Declaration of Interest form (Annexure A)
- • Bidder's Disclosure form SBD 4 (Annexure B)
- • Financial and Technical/Functional Response submitted separately
- • Destruction certificate or guarantee of data formatting
- • Asset removal register compilation with SABC staff
