- • Proof of company office address (municipal account not older than 90 days, OR municipal clearance certificate, OR lease agreement, OR proof of address with affidavit from village residents)
- • For Joint Ventures/Consortiums: separate Tax Clearance Certificate for each party
- • Declaration of Interest form must be completed
- • Company registration proof and company composition form (Annexure E)
- • Reference Assessment Forms from independent references (completed by references and attached to bid)
- • Proof of insurance or indemnification within 14 days of contract award
- • All bid documents completed in black ink and signed
- • Original bid documents (no photocopies)
- • Complete pricing schedule (failure to complete renders bid non-responsive)
