City of Ekurhuleni seeks a panel of service providers for medical surveillance and health risk management services to employees on an as-and-when-required basis until 30 June 2029, evaluated on 80 points for price and 20 points for specific goals.

What you need to qualify for this tender
- • Municipal account not older than three months or signed lease agreement or affidavit
- • No municipal rates/taxes arrears exceeding three months (or proof of payment arrangement) for bids under R10m VAT
- • No municipal rates/taxes arrears exceeding 30 days (or proof of payment arrangement) for bids over R10m VAT
- • Not listed on Register of Bid Defaulters
- • No conflict of interest with state employees or consultants
- • Three annual financial statements if transaction value exceeds R10m VAT (or all statements if established within past three years)
- • All mandatory forms completed and signed: Form of Bid, Form A, Form B, Form C (if applicable), Form D, Form E, Form G, Form H (if applicable)
- • Not previously failed to perform satisfactorily on municipal or state contracts
