- • Proof of manufacturer/supplier agreement if not manufacturer
- • Sample provision and clinical evaluation presentation
- • Material Safety Data Sheets (MSDS) for hazardous chemical substances
- • Product packaging compliance (labelling with manufacturer details, item code, dates, batch/lot number, sterilisation details)
- • Completed WCBD 3.1 pricing schedules on Excel spreadsheet (memory stick and printed copy)
- • Completed evaluation forms for each sample
- • Proof of sample delivery with signatures
- • Compliance with Occupational Health and Safety Act 1993 and Hazardous Chemical Substances Regulations 1995
