- • SBD 1 (Invitation to bid) - fully completed and signed
- • SBD 2 (Tax Clearance Certificate) - original and valid
- • SBD 3.3 (Pricing Schedule) - fully completed and signed
- • SBD 4 (Declaration of Interest) - fully completed and signed
- • SBD 6.1 (Preference points on specific goals) - fully completed and signed
- • SBD 8 (Declaration of Bidder's past supply chain management practices) - fully completed and signed
- • SBD 9 (Certificate of Independent Bid Determination) - fully completed and signed
- • Certified copies of identity documents for all current shareholders/members
- • VAT Registration Certificate (if applicable)
- • CSD Registration Proof
- • Two-envelope submission system (Envelope A: Technical, Envelope B: Financial)
- • Certification stamp on certified documents must be original and no more than 3 months old as at closing date
- • All certified documents must be within current 6 months
- • Joint Venture written agreement with roles/responsibilities if applicable
- • Joint Venture resolutions authorizing signatory if applicable
