- • Company registration documents (Pty/Trust/CC)
- • B-BBEE certificate from SANAS-accredited agency or sworn EME/QSE affidavit
- • Medical reports from qualified practitioner (pre- and post-training)
- • Proof of payment for PCO licence application with Department of Agriculture
- • Sworn affidavits from supervisors (commissioned by Commissioner of Oaths)
- • Company letterhead quotation
- • Latest tax clearance
- • ID copies of company directors
- • SBD forms (all declaration forms)
