Overberg Water invites proposals from registered occupational health practitioners to provide medical testing services for approximately 77 employees over 36 months, evaluated 80/20 on price and BBBEE status with mandatory HPCSA registration and COIDA certificate.

What you need to qualify for this tender
- • SBD1 form (signed, dated within bid advert period)
- • SBD3.1 pricing schedule (quoted on all services, VAT inclusive)
- • SBD4 form (completed in full)
- • SBD6.1 form (completed in full, aligned to CSD)
- • Proof of HPCSA registration with medical council
- • Valid MBChB qualification or recognized occupational health qualification
- • Company profile (detailed)
- • CIPC Certificate
- • Certificate of authority for signatory
- • Copy of authorized signatory's ID document
- • Reference letters (minimum 1 relating to medical services for water board/public entity/corporate/government)
- • Misrepresentation of information will result in disqualification
- • Bid valid for 120 days from closing date
- • No advance payments will be made
