- • Three-year audited comparative financial statements (if procurement exceeds R10M)
- • Letter of Authority to sign bid
- • MBD forms: MBD 4, MBD 5, MBD 7.1 (Part 1 & 2), MBD 8, MBD 9
- • Schedule 1: Resolution of board of directors
- • Schedule 2: Resolution to enter consortia or JVs
- • Schedule 3: Commitments of tenderer
- • Schedule 4: Record of addenda
- • Schedule 5: Compulsory enterprise questionnaire
- • Schedule 6/7: Certificate for water & lights and tenderer's experience
- • Minimum 60% functionality score in Stage Two evaluation
- • Municipal rates not in arrears more than 90 days
- • Not on National Treasury's Restricted Supplier or Tender Defaulter Report
- • Not a person in service of the state or connected to state employee
