- • Certificate of independent bid determination (MBD 9)
- • Declaration of bidder's past supply chain management practices (MBD 8)
- • Declaration of Interest (MBD 4)
- • Pricing schedule with firm prices (MBD 3.1)
- • Form of Offer & Acceptance
- • Formal Contract (MBD 7.1) to be signed within 30 days of acceptance
- • Completed bid documents in original black ink, not retyped, all pages initialed by authorized signatory
- • No municipal rates/taxes arrears exceeding three months; directors must also be compliant
- • Foreign suppliers must complete Part B:3 questionnaire if applicable
