- • SBD 1 fully completed and signed
- • SBD 4 Declaration of Interests fully completed and signed
- • SBD 6.1 Preference Points Claim Form fully completed and signed
- • SANAS accredited BBBEE Certificate or Sworn Affidavit of Measured Entity
- • Valid reference letters with appointment letters/POs/completion certificates for projects completed in last 5 years
- • CV and certified qualifications for key personnel (NQF Level 6 minimum)
- • Health and Safety Plan methodology and CV of H&S manager
- • Proof of access to funding (bank statement or loan letter)
- • Valid lease agreement or municipal account (not older than 3 months) if claiming Free State locality points
