- • Valid Tax Clearance Certificate (valid on closing date)
- • Company Registration documents
- • Copies of Directors ID documents
- • Proof of Bank Account (cancelled cheque or bank letter)
- • Joint Venture/Consortium agreement (if applicable)
- • All RFQ documentation, declarations, and Standard Bidding Documents (SBD) completed and signed
- • Audited Annual Financial statements or B-BBEE Certificate/Affidavit
- • CIPC Documents or B-BBEE certificate/Affidavit
- • CVs for all proposed key personnel with qualifications, HPCSA registration, occupational health qualifications, and years of relevant experience
- • Evidence of previous contracts/projects in occupational health services (appointment letters and reference letters from clients on letterhead)
