- • Item-by-item checklist on official company letterhead with contents, sizes, quantities
- • Current GHS-aligned Safety Data Sheet (SANS 11014) for wound cleaner/antiseptic and other chemical products
- • Product/information datasheet for every item offered
- • SAHPRA registration/licence certified copy (within 3 months) or written authorization letter from licence holder or exemption declaration for Item 1 (antiseptic)
- • Signed statement confirming regulatory status of all non-sterile items - SAHPRA exemption basis or non-regulated status with specific exemption basis cited
- • Physical sample first aid kit fully assembled with all prescribed contents for Stage B practical evaluation
- • Sample must be submitted free of charge and will not be returned
- • All declarations must be on issuer's official letterhead, signed and dated by authorized representative with full name and designation
- • All certified documents must be certified within 3 months before tender closing date
- • Minimum 70% threshold for Stage A (desktop evaluation) and 100% for Stage B (practical evaluation) to qualify
