- • Declaration of Interest (Annexure F)
- • Bid Declaration (Annexure G)
- • Declaration of Bidders past procurement practices (Annexure H)
- • All bid documentation duly signed in ink with initials on all pages
- • Supplier Accreditation Form (SAF) for each Joint Venture/Consortium entity
- • Supporting documents for HDI/RDP preference points if claiming (ID documents, CSD reports, medical certificates, turnover statements)
