- • MBD 5 signed declaration for any offer amounting to or exceeding R10 million
- • Audited financial statements for past three financial years if CIDB Grade 6 or higher and work allocation exceeds R10 million
- • Audited financial statements for past three financial years for any offer amounting to or exceeding R10 million (inclusive of VAT)
- • Latest municipal rates statement (not older than three months) or equivalent proof
- • Certificate of Independent Bid Determination (MBD 9)
- • Declaration of Bidder's Past Supply Chain Management Practices (MBD 8)
- • All MBD forms 1-9 completed in full
- • Joint Venture agreement (if applicable) with comprehensive documentation
- • Municipal rates statement of director's properties
- • Certified copies not older than six months
- • Bid document completed in black ink (not typed)
- • Sealed bid with SCM Number and Project Name clearly marked
