- • SBD 4 (Declaration of Interest) fully completed and signed
- • SBD 6.1 (Preference Points Claim Form) fully completed and signed
- • Tax Compliance Status (TCS) or PIN from SARS
- • Proof of locality in Free State or Northern Cape (municipal rates statement, lease agreement, or municipal clearance certificate not older than 3 months)
- • For ownership claims: Certified ID copies and CIPC registration/CSD reports for Black Economic Empowerment, Women, Youth, or Persons with Disability categories
