- • Certificate of Authority signed by Board of Directors on company letterhead
- • MBD4, MBD6.1, MBD8, MBD9 forms
- • 5 completed ICT audit projects within previous 5 years (functionality threshold)
- • Project Manager with B-degree in Internal Auditing/Accounting plus CIA/CISA with minimum 7 years internal auditing experience
- • CV and certified copies of qualifications
- • Certified ID copies
- • Certified professional accreditation certificates
- • Project methodology documentation (Planning, Verification/Testing, Reporting, Filing phases)
- • Audit file creation and maintenance capability
- • Minimum 50% score per evaluation segment (mandatory)
- • Minimum 40 points (53.33%) on functionality threshold (mandatory)
- • 100% accreditation score (compulsory)
