- • Proof of modification/conversion to crane truck OR signed letter of intent to lease with proof of certificate and modification
- • Roadworthy certificate for vehicle
- • Valid inspection certificate for hydraulic platform vehicle
- • Fully completed MBD forms (MBD 1-9)
- • Full CSD Report (not older than one month)
- • MBD 6.1 for specific goals points (EME/QSE ownership proof)
- • Municipal Rates or Municipal Confirmation letter or Lease Agreement for local business points
- • Proof of non-listing on Register of Tender Defaulters
- • Proof of no previous supply chain management system abuse or contract default
