- • Proof of accreditation if claiming to be accredited representative
- • SBD 1, SBD 4, and SBD 6.1 forms completed and signed
- • Written contract form (SBD7) required post-award
- • Disclosure of state employment, conflicts of interest, and related enterprises (SBD 4)
- • No persons in service of state as directors/trustees/shareholders/members
