- • All quotes on PDF format on company letterhead (MS Word, Excel, pictures not allowed)
- • Valid lease agreement or municipal account statement not older than 3 months
- • Municipal rates and taxes account (not in arrears for more than 90 days)
- • Signed Declaration of Interest form (MBD 4)
- • Declaration of Bidders past supply chain management practices (MBD 8)
- • Certificate of Independent Proposal Determination
- • All MBDs duly completed and signed
- • Service provider must wear PPE (gloves – at least PVC) when collecting bins
- • Registration with CSD (Central Supplier Database) required for login on e-tender portal
