- • Municipal rates and taxes statement (not older than 90 days, not in arrears)
- • Signed Declaration of Interest form (MBD 4)
- • Declaration of Bidders past supply chain management
- • Valid lease agreement or municipal account statement (not older than 3 months, not in arrears for more than 90 days)
- • Datasheet and reference letters demonstrating compliance with specification
- • Manufacturer and model identification with official brochure or datasheet
- • Quotation on two six-monthly calibrations per unit with traceable calibration certificates
- • All quotes on PDF format on company letterhead (MS Word, Excel, pictures not allowed)
- • Joint Venture Agreement where applicable (properly signed by all parties)
- • No RFQ from persons in service of the state
- • Not blacklisted by National Treasury or any National Authority
