- • Valid lease agreement or municipal account statement not older than 3 months
- • Signed Declaration of Interest form (MBD 4)
- • Declaration of bidders past supply chain management practices (MBD 8)
- • Certificate of Independent Proposal Determination (MBD 9)
- • Preference points claim form (MBD 6.1)
- • Company registration documents with ID copies of directors/shareholders
- • Quotation on company letterhead in PDF format only
- • Bill of Quantities/Specification fully completed
- • Datasheet, reference letter, proof of certification (where applicable)
- • All quotations must include all applicable taxes (VAT at 15%)
- • Quotation valid for 60 days from closing date
