- • Signed MBD 4 (Declaration of Interest)
- • Signed MBD 8 (Declaration of past supply chain management practices)
- • Signed MBD 9 (Certificate of Independent Proposal Determination)
- • Proof of CSD registration or MAAA Supplier Number
- • MBD 6.1 (Preference points claim form per PPPF 2022)
- • Company registration documents with ID copies of directors/shareholders
- • Quotation on company letterhead in PDF format only
- • Include brand names where required
- • All applicable taxes included in total quotation value
- • Datasheet/reference letter/proof of certification (where applicable)
