- • Municipal Statement of accounts not older than 3 months OR Lease agreement OR letter from Ward Council confirming proof of address
- • Proof of registration with Central Supplier Database (CSD) full report
- • Separate quotation on company letterhead
- • Completed SBD 4 (Declaration of Interest)
- • Proof of ICT equipment reseller registration with reputable distributors
- • SBD 1 and SBD 3.1 amounts must match exactly
