- • Original or certified copy of B-BBEE certificate or Sworn Affidavit with SANAS logo visible
- • Completed and signed SBD 4 form
- • Completed and signed SBD 6.1 form
- • Central Supplier Database (CSD) report or Unique Number or Supplier number from CSD
