- • Proof of medical certificate confirming disability of owner(s) for preferential points
- • Contactable traceable reference letters from clients (minimum 1 company within last 10 years)
- • CV of ITSM Consultant with minimum 5 years continuous experience in cloud-based ITSM solutions
- • Quotation on official letterhead with total fixed amount including all applicable taxes
- • SBD4 Bidder's Disclosure form
- • SBD 6.1 Preference Points Claim Form
- • POPIA consent form
- • Detailed pricing schedule with per-component breakdown
