- • Two (02) CVs of Technician with Qualifications on Audio Visual System Technology and Polycom Certification + Training
- • SBD1 (Invitation to Bid) fully completed
- • Quotation on company letterhead
- • SBD4 (Bidder's disclosure) fully completed
- • SBD6.1 with original or certified B-BBEE Status Level Verification Certificate or B-BBEE Sworn Affidavit
- • SBD7.2 Contract Form fully completed
- • Proof of Central Supplier Database (CSD) registration
- • Tax Compliance Status (TCS) PIN or CSD number
- • Not listed on Register for Tender Defaulters or List of Restricted Suppliers
- • Not employed by the state (bidder, directors, trustees, shareholders, members, partners, or persons with controlling interest)
