- • EME (annual turnover < R10m) may submit sworn affidavit confirming annual revenue and black ownership level
- • QSE (51%+ black beneficiaries) may submit sworn affidavit confirming annual revenue and black ownership level
- • Confidentiality and Non-Disclosure Agreement (Annexure A) - duly signed
- • Acceptance of Bid Conditions and Bidder's Details (Annexure B) - duly completed and signed
- • Shareholder Information (Annexure C)
- • Bidder's Experience and Proposed Project Team (Annexure D)
- • SBD 1 (Invitation to Bid) - entire RFP document filled and signed
- • SBD 4 (Disclosure and Declaration)
- • SBD 6.1 (Specific Goal Declaration Form)
- • Executive Summary (Schedule 1) explaining understanding of requirements and proposed solution
- • Technical Proposal (Schedule 3) in line with Technical Evaluation Criteria
- • Financial/Price Proposal (Schedule 4)
- • For Consortium/Joint Venture: valid consolidated B-BBEE certificate or Affidavit and signed Consortium/Joint Venture agreement
- • For Prime Contractor with Subcontractor(s): Prime Contractor and Subcontractor(s) B-BBEE certificates or Valid Sworn Affidavits and signed subcontracting agreement
- • Proof of tax compliance with SARS (CSD report)
- • All four schedules must be completed for submission
