- • EME with annual turnover <R10m requires sworn affidavit confirming annual revenue and black ownership level
- • QSE with 51%+ black beneficiaries may obtain sworn affidavit confirming annual revenue and black ownership
- • Confidentiality and Non-Disclosure Agreement (Annexure A) signature required
- • Acceptance of Bid Conditions and Bidder's Details (Annexure B) completion required
- • Shareholder Information (Annexure C) submission required
- • Bidder's Experience and Proposed Project Team (Annexure D) submission required
- • SBD 1 (Invitation to Bid) completion and signature required
- • SBD 4 (Disclosure and Declaration) submission required
- • SBD 6.1 (Specific Goal Declaration Form) submission required
- • Executive Summary explaining RFP understanding and proposed solution
- • Technical Proposal aligned with Technical Evaluation Criteria
- • Financial/Price Proposal
- • For Consortium/Joint Venture: consolidated B-BBEE certificate or Affidavit plus signed consortium/JV agreement
- • For Prime Contractor with Subcontractors: B-BBEE certificates/Affidavits for all parties plus signed subcontracting agreement
- • All four (4) Schedules must be completed for bid submission
- • Proposal validity period of at least 150 days after closing date
