- • Registered on Central Supplier Database (CSD)
- • SARS PIN or Tax Clearance Certificate (original, valid for 1 year)
- • Certified B-BBEE Certificate from registered accountant OR original Sworn Affidavit from Commissioner of Oath
- • Valid company registration document
- • Certified current account (water/electricity/rates/taxes) not older than 3 months OR lease agreement
- • Certified copies of identity documents of directors and owners
- • SABS Approved Certificate for protective clothing and all other requirements
- • Proof of payment for bid document (R2000 or R1500 for electronic copy)
- • MBD forms completed (MBD2, MBD3.1, MBD4, MBD6.1)
- • Questionnaire for foreign suppliers (if applicable)
- • Not in service of state
- • No relationship with persons in service of state involved in evaluation
