- • Audited financial statements for past three financial years (for offers above R10 million inclusive of VAT)

- • Record of Addenda to Bid Documents
- • Compulsory Enterprise Questionnaire
- • Declaration of Interest (MBD 4)
- • Declaration for procurement above R10 million (MBD 5)
- • Preference Points Claim Form (MBD 6.1)
- • Declaration of Past Supply Chain Management Practices (MBD 8)
- • Certificate of Independent Bid Determination (MBD 9)
- • Tertiary qualification in digital media/services
