MANDATORY REQUIREMENT • Fully completed and signed SBD1 must be attached, • Fully completed and signed SBD4 must be attached, a false declaration will result in disqualification (par. 2.4 disclosure must align with subsection 2.4.1 and director‘s information on Central Supplier Database CSD) • Fully completed and signed SBD6.1 must be attached. • Bidder/ s must submit the bid to the tender box to address indicated below and on SBD1, • Bidder/s must attach appointment letter in the SITA Transversal contract (SITA RFB 1183/2022). • Pricing Schedule must be fully completed and attached. • Annexures (B4.1); (B4.2); (B4.3); (B5.1); (B5.3); B5.4) must be fully completed and attached
