- • Recent municipal account (not older than 90 days)
- • Recent municipal accounts of all Directors/Shareholders
- • If leasing/renting property: valid copy of agreement
- • If no property registered: affidavit confirming this
- • MBD forms if quotation value exceeds R30,000.00 (MBD 4, MBD 6.1, MBD 7, MBD 8, MBD 9, MBD 10)
- • Declaration of Interests (MBD 4)
- • Certificate of Independent Proposal Determination (MBD 8)
- • Certificate for Payment of Municipal Services (MBD 9)
- • Declaration of Bidders Past Supply Chain Management Practices (MBD 10)
- • Not a person in the service of the state
