- • BBBEE Certificate (certified copy or sworn affidavit for QSE/EME)
- • Municipal Account Statement (bidder and all owners/directors/shareholders) not older than 3 months, not in arrears >90 days
- • CSD summary report
- • Audited Financial Statements for most recent 3 years (or from establishment if <3 years old)
- • Completed MBD forms (MBD 1, 4, 5, 8, 9) in black ink, fully signed
- • Authorization letter if signatory not director/member/owner
- • Technical Schedule B completed in full in black pen
- • Pricing Schedule completed for all items in black ink with initialled corrections only
- • Joint Venture agreement (if applicable) signed by all parties with all supporting documents for each JV partner
- • Sample of the poles before delivery approval
