- • Declaration of Interest (MBD 5)
- • Declaration of Past Supply Chain Management Practices (MBD 8)
- • Not in service of the state
- • Not listed on Register of Bid Defaulters or National Treasury prohibited list
- • No outstanding municipal rates/taxes in arrears for more than 3 months
- • Satisfactory performance on previous municipal/state contracts in past 5 years
- • No fraud or corruption conviction in past 5 years
- • No willful breach of government/public sector contracts in past 5 years
