- • Detailed CVs with original certified copies of Lifeguard Award Certificates (not older than 6 months)
- • Proof of ownership of lifesaving equipment (whistles, rescue boards, fins, flags, torpedoes, first aid boxes)
- • Project methodology demonstrating management, risk management, and turnaround times
- • Certificate of Independent Bid Determination (MBD 9)
- • Declaration of Conflict of Interest (MBD 4)
- • Declaration of Past Supply Chain Management Practices (MBD 8)
- • Signed contract form (MBD 7.1)
- • Not listed on Register of Tender Defaulters
- • No abuse of Supply Chain Management System or previous contract failures
- • Municipal tax clearance (rates/taxes not in arrear for more than 3 months)
