- • Full Central Supplier Database (CSD) report not older than one month
- • MBD 6.1 Preferential Points Schedule fully completed and signed
- • Proof of South African ownership (for 10-point specific goal)
- • Proof of business location in Mnquma Local Municipality area - Municipal Rates, Municipal Confirmation letter, or Lease Agreement (for 10-point specific goal)
- • All returnable MBD forms (MBD 1-9) completed
- • Certificate of Independent Bid Determination (MBD 9)
- • Declaration of Tenderer's Past Supply Chain Management Practices (MBD 8)
- • Conflict of Interest Declaration (MBD 4)
- • For partnerships/consortiums/joint ventures: signed agreement and all parties registered on CSD
- • Company Registration documents and certified ID copies not older than six months (for preferred bidders)
- • Municipal rates clearance certificate or confirmation of no municipal arrears exceeding three months (for preferred bidders)
