- • Schedule 1: Resolution of board of directors
- • Schedule 2: Resolution to enter consortia or JVs
- • Schedule 3: Schedule of proposed sub-contractors
- • Schedule 4: Commitments of tenderer
- • Schedule 5: Record of addenda
- • Schedule 6: Compulsory enterprise questionnaire
- • Schedule 7: Certificate for water and lights
- • Schedule 8: Tenderer's experience
- • All MBD forms (MBD 1, 2, 3.1, 4, 6.1, 7.2 Part 1 and 2, 8, 9)
- • Duly completed and signed offer/quotation
- • Declaration of independence in bid determination
- • Proof not in service of the state
- • Functionality score minimum 60% to progress to price evaluation stage
