- • Certified copies of all directors' IDs (not older than 3 months)
- • Full comprehensive CSD registration report (not older than 30 days)
- • Certified copy of current municipal account or valid lease agreement
- • Declaration of municipal rates not in arrears more than 90 days
- • Valid B-BBEE certificate (SANAS verified or sworn affidavit for EME/SME)
- • MBD 1 – Invitation to Bid form
- • All MBD Declaration forms (MBD 2, 4, 6.1, 7.2, 8, 9)
- • Duly completed and signed MBD 1 with formal written price quotation
- • Schedule 1 – Resolution of Board of Directors
- • Schedule 2 – Resolution to enter consortia/JV
- • Schedule 3 – Schedule of proposed sub-contractors
- • Schedule 4 – Commitments of tenderer
- • Schedule 5 – Record of addenda
- • Schedule 6 – Compulsory enterprise questionnaire
- • Schedule 7 – Certificate for water & lights
- • Schedule 8 – Tenderer's experience
- • Not in service of the state (bidder or any director/shareholder)
- • Not listed on National Treasury's Restricted Supplier/Tender Defaulter database
- • Registered on CSD prior to submission
