- • Completed SBD 4 form duly signed
- • Completed Enterprise Questionnaire
- • Resolution of signatory form or director authorization letter
- • Not listed on Register of Tender Defaulters
- • No previous abuse of Supply Chain Management System or contract performance failures
- • No conflicts of interest declaration
- • Tax matters in order with SARS (verified through CSD)
- • Original plus one copy plus one scanned copy on memory stick
