- • No previous abuse of Supply Chain Management System
- • No failed performance on previous contracts
- • Financial documentation: letter of intent from NCR-accredited institution (max 3 months old), or proof of overdraft facility, or proof of self-funding capability (bank statement max 3 months old)
- • Reference letters proving occupational hygiene services experience (minimum 3 letters required for full points)
- • Appointment letters and/or purchase orders for previous occupational hygiene projects
- • Proof of residence if company located outside Limpopo Province (municipal bill/PTO/tribal authority letter)
- • Medical Certificate from Medical Doctor or Hospital if claiming disability ownership points
- • Proof of Registration on National Military Veterans Database if claiming military veteran ownership points
