- • Preference points claim form (SBD 6.1) if applicable
- • All bid documents must be initialed on each page
- • Pricing schedule (SBD 3) with firm prices for all five years
- • For consortia/joint ventures: separate TCS certificate and CSD number for each party
- • Sealed envelope submission with bidder name, address, bid number, and closing date on envelope
- • Completed bid document in official forms (not retyped)
- • No persons in service of the state as directors, members, or shareholders
