- • Minimum 15 out of 25 functionality points required (3+ projects completed in last 5 years with performance assessments)
- • JGDM Performance Assessment Forms signed and stamped by previous clients for each submitted project
- • Proof of company registration and/or legal standing
- • Declaration of Interest form completed
- • Company composition form (Annexure E) completed
- • Original bid documents with all pages signed
- • Detailed fee structure for 8 types of employment checks (VAT inclusive)
- • Proof of company office address for locality component (municipal account not older than 90 days, OR municipal clearance certificate, OR lease agreement, OR proof of address with affidavit)
- • Turnaround time commitment of no longer than 5 working days for all check types
- • Identity documents of directors, CSD form, and company registration documents for HDI claims
- • Not a person in service of the state
