- • Valid COIDA Certificate from Department of Labor (Letter of good standing)
- • Proof of address (latest municipal statement not older than 3 months, not in arrears >90 days, or valid lease agreement)
- • Company shareholder certificate(s)
- • ID copy of owner(s) of company
- • Joint Venture agreement (if applicable) signed by all parties with all mandatory documents
- • Completion of Bill of Quantities/Specification in full
- • Completion of MBD forms: MBD 1, MBD 3.1, MBD 4, MBD 6.1, MBD 8, MBD 9
- • Functionality pre-qualification minimum threshold: 80 points out of 100
- • Reference letters (minimum 3) from clients where MPLS design and implementation services were rendered
- • Detailed project management methodology and implementation schedule
- • Service Level Agreement proposal with specified parameters (Hardware Service Availability 98.30%, Core Infrastructure Availability 99.95%, Hosting Infrastructure Availability 99.95%, penalties, preventative maintenance schedule, business continuity plans)
- • CV of CRM Service Management personnel for account and service management
- • Qualified network architect with: Diploma in Computer Science/IT, OR Bachelor's/BTech in Computer Science/IT, OR Honors+ in Computer Science/IT, OR Cisco Certified Network Professional (CCNP Enterprise) certification (not older than 3 months from tender closing date)
- • Proof of affiliation with authorised official bodies (ICASA and ISPA)
