- • ID copy of owner(s)
- • Valid Joint Venture agreement if applicable (with specified requirements)
- • CSD registration with vendor number
- • Completion of Bill of Quantities/Specification
- • MBD 1, MBD 3.1, MBD 4, MBD 6.1, MBD 8, and MBD 9 forms
- • All corrections must be initialed by bidder
- • No correction fluid allowed
- • Sealed envelope submission with clear marking
