- • Proof of payment if bid documents purchased from municipality
- • MBD1, MBD2, MBD3.1, MBD3.2, MBD4, MBD6, MBD6.1, MBD8, MBD9 completed and signed
- • Declaration that bidder not in service of state
- • Certificate of Independent Bid Determination (MBD9)
- • Declaration of past supply chain management practices (MBD8)
- • All pages signed and witnessed by two witnesses where required
- • Local content compliance for transformers (70%)
- • Audited annual financial statements for past 3 years (if project >R10 million)
- • Particulars of contracts awarded by organs of state in past 5 years (if >R10 million)
- • Statement on foreign sourcing of goods/services and payment transfers (if >R10 million)
