- • Tax Compliance Status PIN from SARS or printed TCS certificate
- • Central Suppliers Database (CSD) registration report (full report, not summary)
- • Declaration of Interest (MBD 4) - original signed
- • Declaration of bidder's past supply chain management practices (MBD 8)
- • Certificate of Independent Bid Determination (MBD 9)
- • Proof of payment if bid documents purchased from municipality
- • Medical Certificate if claiming specific goal for disability
- • Local Content Compliance - 90% for Electrical Cables (MBD 6.2 if required)
- • Audited Annual Financial Statements for past 3 years (if contract value expected to exceed R10 million)
- • Particulars of contracts awarded by organs of state in past 5 years (if contract value expected to exceed R10 million)
- • Statement on sourcing of goods/services from outside RSA (if contract value expected to exceed R10 million)
- • Completed Listing Criteria
- • Original and duplicate bid documents in sealed envelopes
- • All pages initialled in original submission
- • Two witness signatures on declaration forms
