- • MBD1 (Invitation to Bid) must be completed and signed
- • MBD2 (Declaration of Interest) must be completed and signed
- • MBD3.1 (Pricing Schedule and Specifications) must be completed
- • MBD3.2 (Method of Pricing and Price Adjustments) must be completed
- • MBD4 (Declaration of Interest) – original signed, no state employees
- • MBD6.1 (Valid BBBEE certificate or sworn affidavit for EMEs/QSEs)
- • MBD6.2 (DTI standards/local content compliance if required)
- • MBD8 (Declaration of bidder's past supply chain management practices)
- • MBD9 (Certificate of Independent Bid Determination)
- • CSD registration report (current, full report, not summary)
- • Current municipal account (not older than 90 days) or valid lease agreement
- • Valid tax compliance status PIN or TCS certificate from SARS
- • Medical certificate if claiming disability-specific goals
- • Listing Criteria must be completed
- • One original bid and one duplicate in separate sealed envelopes
- • All pages must be signed and witnessed by two witnesses where required
- • Proof of payment if bid documents purchased from municipality
- • For contracts >R10 million: audited annual financial statements for past 3 years or since establishment; particulars of contracts awarded by organs of state in past 5 years; statement on foreign sourcing and payment transfers
- • Bidder must confirm not in service of state in previous 12 months
- • Foreign suppliers must complete Part B questionnaire (Paragraph 5)
