- • MBD 4 (Declaration of Interest)
- • MBD 6.1 (BBBEE certificate or sworn affidavit)
- • MBD 8 (Declaration of bidder's past supply chain management practices)
- • MBD 9 (Certificate of Independent Bid Determination)
- • Listing Criteria completed and signed
- • Original and duplicate bid documents in separate sealed envelopes
- • All pages signed and initialed as required
- • Two witnesses signature and names on documentation
- • Local content compliance: Steel must be 100% local
- • For bids exceeding R10 million: Audited annual financial statements for past 3 years or since establishment, particulars of state contracts awarded in past 5 years, statement on sourcing outside RSA
