Appointment of 20 service providers to supply pothole repair materials (hot asphalt, cold asphalt, cement, emulsion, crusher dust) to Free State Department of Community Safety, Roads and Transport for 36 months; requires CIDB Grade 5 or 6 CE, valid tax PIN/CSD, and demonstrated experience in roads commodity supply.

What you need to qualify for this tender
- • Fully completed and signed SBD forms (SBD 1, SBD 3.1, SBD 4, SBD 6.1)
- • All returnable schedules (A through L)
- • CSD registration number or CSD printout
- • CIPC registration copy
- • Certified copies of ID documents of all active members/shareholders (not older than 3 months, certified by SAPS)
- • Copy of valid Tax Clearance PIN from SARS
- • Copies of Purchase Orders and Reference Letters for prior commodity supply/delivery work (not older than 01 January 2016)
- • Bank guarantee letter demonstrating financial capacity to sustain service delivery for 2 months
- • Bank rating not older than 6 months
- • SABS-certified laboratory test reports for all commodities supplied
- • B-BBEE Status Level Verification Certificate or Sworn Affidavit (for EMEs and QSEs, to qualify for preference points)
- • Proof of authority if bid signed by representative (e.g. company resolution)
